Opiate Dependence Treatment in Primary Care:

Shining Life/Hutto Wellness is here to proving a resource for those who are dedicated getting control over their opiate dependence and back to spending time with their loved ones. Much of the medical community agrees that opiate dependence is lifelong condition and requires long term treatment. There are patients in practice who have been able to wean themselves off treatment over time however this is not often the case. Therefore, Acute Treatment Programs are often just the beginning of a journey to regain control your life.
What Can Happen with Opiate Use:
Opiate use from both prescribed and illicit sources often leads to physical dependence. Opioid dependence often causes withdrawal symptoms including:
• Agitation
• Anxiety
• Muscle aches
• Insomnia
• Abdominal cramping
• Diarrhea
• Nausea
• Vomiting
These symptoms often interfere with a person’s family life, social interactions, work, and over all their life’s potential.     Medical treatment:

    • Buprenorphine: is an opioid medication used to treat opiate dependence and some chronic pain. This is by far the best medication to treat opiate dependence available to date. Patients who are taking Buprenorphine can take this medication and go about their daily lives. The somnolence, disorientation, and mental confusion produced by its predecessor methadone, are absent or not significant and in most cases. Patients taking Buprenorphine are Business Owners, Teachers, Parents, and play many more vital roles within our communities. We only Prescribe Buprenorphine with Naloxone formulations.
    • Naloxone: is a medicine that rapidly reverses an opioid overdose. It is an antagonist. This means that it attaches to opioid receptors and reverses and blocks the effects of other opioids. Naloxone can quickly restore normal breathing to a person if their breathing has slowed or stopped because of an opioid overdose.
    • Buprenorphine and Naloxone (Suboxone): is the brand name medication which includes Both Buprenorphine and Naloxone. The Naloxone portion of Suboxone is a precautionary measure to prevent alteration of this medication. The Naloxone in Suboxone is formulated so that it is not bioavailable and will have no action unless it is altered and injected directly into the vein. Patients who are taking Suboxone, only receive benefit from the Buprenorphine portion of the medication as the Naloxone remains inactive unless the medication is altered.

    Opiate Dependence Treatment Program Guidelines:

      The Initial Visit

      Your initial visit will cost $75 can be done in the Office or by Telehealth through the end of 2025 according to current DEA regulations. The initial visit will include a Urine Drug Screening for a cost of $15 in the clinic. Alternate collection at any of your local Lab Corp locations is an option for Telehealth Patients however is a bit more expensive at $55.

      Follow Up Visits

      Monthly follow up visits will cost $75 and are required to ensure safety and to monitor progress. Urine Drug Screenings are required every 3 months and more frequent if positive for illicit substances.

      Drug Screening

      If your initial drug screening is positive, we recognized that you are seeking our assistance to change from the illicit market to a safer alternative. Depending on your history we will prescribe a two-week supply of Buprenorphine/Naltrexone. In some case with a recorded Buprenorphine/Naltrexone prescription history on the PDMP website, you may receive a 30-day prescription at your initial visit. If your urine drug test comes back positive at any time, you will be required to come back in 2 weeks and repeat the drug screen to show that you have stopped using the illicit substance as well as ensure that you are taking the prescribed medications. If you have been prescribed another controlled substance let us know during your visit. We will also see your narcotics prescription history during a mandatory review of the Florida Prescription Drug Monitoring Program (PDMP) electronic database during each visit. All positive results for illicit and non-prescribed substances will require an additional fee of $45 to send the urine specimen to the lab to quantify the exact amount of the elicit substance is in your system. Most illicit substances will be significantly reduced or absent after a two-week period. THC can stay in your system for up to 30 days however many people have prescriptions for THC. The legal Delta-8 hemp derived forms for THC will also show positive although no laws have been broken. For these reasons we do not consider THC an illicit substance regarding your treatment.

      Prescription Drug Monitoring Program (PDMP)

      The Florida prescription drug monitoring program (PDMP) is an electronic database that tracks all controlled substance prescriptions prescribed in the state of Florida. It is mandatory for all prescriber to check the PDMP prior to prescribing any narcotic substance. It serves a valuable tool to improve safety. It helps to monitor a patients narcotic prescriptions history. A patient who may be switching form one prescriber to another for personal or financial reasons would show a consistent history with routine refills of their medication. I other cases a patient may be going from one prescriber to the next within a short period of time which would raise questions regarding their sincerity with treatment or something more ominous. The PDMP is an excellent tool for both prescribers who have concerns regarding a patient’s history as well as to help a patient demonstration dedication to their treatment program.
